Requests for certification of property values, identification, etc., are made to assist in legal proceedings,
court actions, financial matters, etc..
The public should be advised that they may obtain and print this information from the public access terminals, or have the
information typed on department letterhead at no charge. These may be stamped "Certified True Copy" at no charge.
However, if a "Certified" document is required, which means formal wording attesting to information, an official
signature, and department seal, then the two part certification form is to be used. A $6.00 fee per document is to be
charged.
The form should be numbered by each office with the second copy kept in office files. The $6.00 fee should be processed in
accordance with cash receipt procedures and coded as Miscellaneous Revenue. (Attachment 1 is a copy of the form to be used
